Online Survey Policy

Due to the increasing demands placed on the School of Pharmacy IT department and the labor intensive
nature of electronic surveys, the IT Department no longer supports electronic surveys.

For simple online surveys we have two recommendations:

  1. www.SurveyMonkey.com

    SurveyMonkey allows individuals to create web accessible surveys and view survey data online for free. To download the data into Excel or SPSS, the user must pay a monthly membership fee. Free SurveyMonkey Surveys can include the following question types multiple choice, multiple answer, open-ended essay, and rating scales. SurveyMonkey also offers choices for background colors. If your survey has HIPPA requirements you will need to investigate if Survey Monkey meets them.

  2. CIT Form Builder

    CIT (Center for Instructional Technology) provides instructions for adapting their free survey form. The form has the following features:

    1. Three types of questions are available:
      1. multiple choice, allowing one response; checkbox,
      2. allowing more than one response;
      3. and text, allowing free-text response.
    2. Custom background colors can be set.
    3. Individual responses are delivered via email, but responses are not recorded in a database.
    4. Anonymous and identified surveys are possible by creating or not creating identifying questions in the form itself. No individualized usernames and passwords are available for entrance into the form.

    If your survey has HIPPA requirements you will need to investigate if the CIT Form meets them. CIT does not store the survey on its own web server. You must have a website where you can upload it.
